Position: LPN - Healthcare Coordinator
> Healthcare Coordinators are responsible for clinical management of our Assisted Living community. The candidate will form lasting relationships with our residents, work a flexible schedule, and experience an environment with a warm, family atmosphere.
Healthcare Coordinator duties:
Provide proper documentation of all nursing and healthcare services
Responsible for maintaining health service plans and resident assessments
Monitor effective communications regarding resident care
Direct the department's safety and loss-prevention program
Supervise, evaluate and train Care Managers / Medication Care Managers and Wellness Nurses
Recruit and retain all team members in the department, ensuring their effective orientation and training
Establish positive relationship programs and practices

Requirements
This role requires a current LPN licensure, at least 2 year of experience, affinity for working with the senior population and the ability to work flexible hours.
