Position Summary:
Provide health care, education and preventive services on Outreach bus, and support the mission and goals of The Night Ministry by offering hospitality, health education, assessments, counseling, medical treatment, and referrals for persons (13+ yrs old) experiencing homelessness and those without access to adequate healthcare . Works two (2) – three (3) evenings per week; some Saturdays required (30 hours per week).

Essential Functions: Conduct basic physical assessments. Administer confidential STD and HIV testing, counseling. Ability to work closely and in a team environment with other staff members, volunteers, and referral staff is essential. Offer referrals for health care. Maintain documentation of health-related activities and other services provided, as required by funding or accrediting sources. Participate in internal and external trainings as required. Able and willing to travel to other outreach settings, such as shelters. Attend all department, agency and inter-agency meetings as required. Complete other duties to support the program and mission of the organization as needed.

Position Requirements: Must possess a M.S.N. degree and current Advanced Practice Nursing License. Must clear all DCFS requirements, including health clearance and criminal background check. Minimum of five (5) years nursing experience. Must have demonstrated experience in counseling patients/clients. Excellent written/oral communication and interpersonal skills required. Excellent judgment, ability to react calmly and appropriately in emergency situations; demonstrated “street smarts.” Must be able to work in various settings, including outdoors. Familiarity with referral sources a plus. Current driver’s license and good driving record; access to reliable personal transportation. Spanish language skills highly desired but not required.
